APPLICANT INFORMATION AND DECLARATION

Before submitting your application, please read the information below. It explains some important matters that may apply to your application and the information you may be asked to provide during the recruitment process.

For information about how AHCWA collects, uses, stores and discloses personal information, please see the Careers Portal Privacy Notice and AHCWA's Privacy Policy.

INHERENT REQUIREMENTS AND SAFE PERFORMANCE OF THE ROLE

Each position has inherent requirements. These are the essential duties, responsibilities and requirements that must be performed to fulfil the purpose of the position.

Depending on the position, inherent requirements may relate to physical, cognitive, communication, interpersonal, environmental or other capabilities.

Where relevant to the position, AHCWA may ask questions or undertake pre-employment checks to assess whether you can safely perform the inherent requirements of the position.

AHCWA will consider reasonable adjustments to the recruitment process and the role where appropriate.

You do not generally need to disclose a medical diagnosis or detailed medical history. Where information about a health condition or disability is relevant to the inherent requirements of the position, a reasonable adjustment or another lawful recruitment requirement, AHCWA may request information that is reasonably necessary for that purpose.

If you require a reasonable adjustment to participate in the recruitment process, please let us know. You can contact the People and Culture Team at People.Culture@ahcwa.org or (08) 9227 1631.

HEALTH AND OTHER SENSITIVE INFORMATION

For some positions, AHCWA may need to collect sensitive information where it is reasonably necessary for recruitment, pre-employment screening, workplace health and safety, legal or regulatory requirements, or other lawful requirements relating to the position.

Sensitive information may include health information, criminal record information, or information about racial or ethnic origin, including Aboriginal and/or Torres Strait Islander identity.

Where your consent is required to collect sensitive information, AHCWA will provide the relevant information and seek your consent as part of the application or recruitment process.

Providing sensitive information does not, of itself, determine the outcome of your application. Information will be considered only where it is relevant to the requirements of the position or the purpose for which it was collected.

WORKERS COMPENSATION CLAIMS

You are not required to disclose information about any current or previous workers compensation claim as part of your application or pre-employment screening.

AHCWA will not ask you to disclose details of a workers compensation claim for the purpose of assessing your suitability for employment.

  • the information you provide is accurate and complete to the best of your knowledge;
  • the information you provide is relevant to the application questions and the position for which you are applying;
  • you will advise AHCWA if you require a reasonable adjustment to participate in the recruitment process;
  • you understand that AHCWA may undertake relevant pre-employment checks where these are reasonably necessary for the position and permitted by law; and
  • you have read and understood the Careers Portal Privacy Notice.

You understand that providing information about a disability, health condition or other sensitive information will not, of itself, determine the outcome of your application. AHCWA will consider relevant information in the context of the requirements of the position and any reasonable adjustments that may be available.
